To be fully enrolled and in order to commence the class, the following information must be submitted to the admissions office:
1. This application together with Payment of $100 Tuition Fees
2. Copy of CMA Licence and Government issued ID
3. Completed Student Affidavit and KDHE Med Aide Renewal Application Form (**This two forms are available on the reference section of this website**)
